4. Have you worked for any musical organizations, piano studios, bands, musical theater, or any other groups affiliated with the teaching, performance or promotion of music?

6. Do you currently have any music projects underway?
7. Why are you a musician?
Take the above information and turn it into your biography: the short history of your life that highlight the things you want the public to know about you, your piano performance or teaching career, and your accomplishments/dreams. Type it out on the computer—it should be no longer than 1 page, single spaced (or double spaced, if you prefer).
